HEART AWARD

What are the Awards? The Arkansas State Hospice & Palliative Care Association Heart Award is designed to honor the person affiliated with your Hospice who goes above and beyond the call of duty, giving of themselves, and enhancing the quality of life for Hospice patients.

Should your nominee not receive the 2010 award they may be held over for the 2011 award with your permission.

 Four categories will be awarded:

           Hospice Employees

           Hospice Volunteers- (can be a team of two)

           Hospice Caregivers

           Hospice Aides

Application Process: Nominations may be submitted by anyone wishing to offer a candidate for consideration.
